Ask the next question only when it matters.

Any question can depend on the answer to an earlier one. People only see what applies to them, so forms stay short and the answers you get are complete.

Outcomes

A different response for each result.

Flag answers green, amber or red. When someone submits, they see the message for their result. Use it for a triage, a safety check or a self-assessment, or just to thank people.

Green

"Everything looks good. You're booked in, and we've emailed the details."

Amber

"A couple of things need attention. Here's what to do before your visit."

Red

"Please call us now on the number below before going any further."

After submit

Every answer ends up where it's needed.

Each form has an action that runs on every submission, and the submission is kept in your admin as well.
